Face it, stuff happens. That's why we would like to provide you with the ability to ring up sales should you have issues accessing the software. Below you will find two files. One is in an Excel file format so you can fill it out electronically and the other is a PDF file so you can print it out and fill it in by hand.
We recommend downloading the PDF and printing off 20 or so copies to have on-hand in case you are unable to access the internet.
